The purpose of this study is to determine empirically the state of the art of the medical care, when healthcare personal is confronted with ethical dilemmas related with the care they give to the geriatric population. An observational, longitudinal, prospective and qualitative study was conducted by analyzing the correlation between healthcare personnel–patient relationship, and ethical judgments regarding dilemmas that arise in daily clinical practice with geriatric patients. Mexican healthcare personnel with current active practices were asked to write up an ethical (...) dilemma that arose frequently or that had impacted their medical practice. From the narrative input, we were able to draw up a database with 421 dilemmas, and those corresponding to patients 60 years and older were selected (n = 54, 12.8 %). The axiological analysis of the narrative dilemmas of geriatric patients was made using dialectical empiricism. The axiological analysis values found most frequently were classified into three groups: the impact of healthcare, the roles of the physician, and refusal of therapy; the healthcare role of educator, caring for the patients’ life and the risk of imminent death where the values found more often. The persistence and universality of certain dilemmas in geriatrics calls for awareness and requires a good training in the ethical discernment of these dilemmas. This would help to improve substantially the care and the life quality of this population. (shrink)

In our time there is growing interest in developing a systematic approach to oncologic patients and end-of-life care. An important goal within this domain is to identify the values and ethical norms that guide physicians’ decisions and their recourse to technological aids to preserve life. Though crucial, this objective is not easy to achieve.

Hrushovski proved that the theory of difference-differential fields of characteristic zero has a model-companion, which we shall denote DCFA. Previously, the author proved that this theory is supersimple. In supersimple theories there is a notion of rank defined in analogy with Lascar U-rank for superstable theories. It is also possible to define a notion of dimension for types in DCFA based on transcendence degree of realization of the types. In this paper we compute the rank of a model of DCFA, (...) give some properties regarding rank and dimension, and give an example of a definable set with finite rank but infinite dimension. This work presents a proposal for the characterization of the UnconfinedCompressive Strength test, through a series of operations that can be carried outwithout inconvenience in the field. Initially, fresh rock samples are obtained from outcropsin the area and specimens of specific dimensions are made. After the test specimenelaboration phase, crushing and granulometric classification tests are carried out witha set of specimens and in parallel with a second group, UCS tests are carried out. With theresults, the rock is characterized by graphing (...) granulometric curves and in this graph theareas of fine granulometry are focused, inserting in these areas, the average value of UCS,with which it can be identified and determined when it is really necessary and It is inevitableto send rock samples to laboratories, thus saving time and money for the mining project. As if I could read the darkness."Philosophical Investigations, §635We live, as we dream—alone."1 This, Marlow's most eminently quotable aphorism, encapsulates a theme central to the outlook of modernism: what Virginia Woolf called "the loneliness which is the truth about things."2 This loneliness derives not from the absence of others—Marlow is surrounded by friends when he makes this assertion. It is a deeper (...) condition brought about by recognizing the limits of selfhood, by confronting or being confronted by them. This condition is central to Marlow's narrative predicament: he attempts to fathom the enigmatic self that is Kurtz, to understand "the tempestuous anguish of his soul" (HoD, p. 246); he is led, by this attempt, to question the nature of his own self; and, finally, he tries to relay the impenetrable essence of both these selves to the "other minds" of his narratees on the yawl Nellie. As we shall see, Marlow concedes defeat in all three of these struggles. Heart of Darkness, then, is a novel beset by the limits of subjectivity.It is also a novel which foregrounds the limits of language. Marlow concludes that we live in isolation from one another after musing on the failure of language to communicate adequately, to facilitate intersubjective understanding: "it is impossible to convey the life-sensation of any given epoch of one's existence" (HoD, p. 172), he says. [End Page 409] Conveying the meaning of his narrative to others is repeatedly written off as unattainable, and on numerous occasions Marlow, presumably wrestling with sheer wordlessness, falls silent.3 Indeed, Marlow's narrative is peppered with words highlighting the limits of language: words like "inconceivable" and "unspeakable," which F. R. Leavis famously dismissed as "overworked."Leavis described Conrad's "adjectival insistence upon inexpressible and incomprehensible mystery" as "worse than supererogatory," "tending to cheapen the tone" to a point "little short of disastrous." He concluded that Conrad was "intent on making a virtue out of not knowing what he means."4 That Leavis—a critic who fetishized "the concrete presentment of incident, setting and image" through the concrete use of words—should have failed to grasp and to value the novel's brush with wordlessness, is unsurprising (Leavis, p. 208). Nor is it surprising that two of the episodes in the novel that most clearly encounter the limits of language—Kurtz's last words and Marlow's lie about them—were written off by Leavis as examples of Conrad having a "bad patch" (Leavis, p. 208). Yet it is nevertheless instructive that a novel that insists on its linguistic and communicative limits should thus have been misread.This article reads these two issues of the limits of language and the limits of subjectivity together. To do so is to invoke a philosophical position in which language, selfhood and worldhood are interrelated, and their limits coterminous—a position found in the early thought of Ludwig Wittgenstein.ILike Marlow, Wittgenstein also sees the self as essentially isolated from other selves, and language as demarcating its limits. In his Tractatus Logico-Philosophicus, the interconnected remarks "The limits of my language mean the limits of my world" and "I am my world" delineate Wittgenstein's rethinking of the "egocentric predicament" in which, to paraphrase Marlow, we all live alone.5 This position entails a close relationship with solipsism: as Wittgenstein says, "what the solipsist means is quite correct" (TLP, 5.62)—which is not, of course, to suggest that solipsism itself is correct. The grammar of Marlow's assertion encapsulates this point. The card-carrying solipsist would assert "I live alone," but the loneliness that Marlow and Wittgenstein envisage is a [End Page 410] condition of our subjectivity that we all have in common, but cannot share. The frequency and severity of natural disasters and extreme weather events are increasing, taking a dramatic economic and relational toll on the communities they strike. Given the critical role that entrepreneurship plays in a community’s viability, it is necessary to understand how small business owners respond to these events and move forward over time. This study explores the long-term dynamics and trajectory of individuals within the broader business community following a natural disaster, paying particular attention to the influence of social (...) identity. Results suggest that the community identity changes over the course of recovery and rebuilding, underscoring the need for a holistic approach so that intervening agencies can achieve the sustainable economic recovery desired. This article focusses on the generality of the entities involved in a geometric proof of the kind found in ancient Greek treatises: it shows that the standard modern translation of Greek mathematical propositions falsifies crucial syntactical elements, and employs an incorrect conception of the denotative letters in a Greek geometric proof; epigraphic evidence is adduced to show that these denotative letters are ‘letter-labels’. We no longer have any texts, any past, but just interpretations of them. The evident multi -interpretability of a text causes it gradually to lose its capacity to function as arbiter in the historical debate. It is necessary to define a new link with the past based on a complete and honest recognition of the position in which we now see ourselves placed as historians. In recent years, many people have observed our changed attitude towards the phenomenon of information. For (...) postmodernism, science and information are independent objects of study which obey their own laws. Language and art are not situated opposite reality but are themselves a pseudo-reality and are therefore situated within reality. Because of the relation between the historiographical view and the language used by the historian to express his view - a relation which nowhere intersects the domain of the past -historiography possesses the same opacity and intensional dimension as art. The essence of postmodernism is precisely that we should avoid pointing out essentialist patterns in the past. There is reason to assume that our relation to the past and our insight into it will in future be of a metaphorical nature rather than a literal one. (shrink)

Este artículo discute los aportes de Quentin Skinner al análisis de las imágenes en la retórica política del Renacimiento y explora una elucidación teórica sobre su funcionamiento argumentativo aplicando la teoría de los actos de las imágenes de Horst Bredekamp. Después de revisar las dimensiones de la retórica de Skinner, el trabajo se enfoca en el examen del lugar que ocupan las imágenes en los argumentos retóricos. Para esto discute algunas imágenes icónicas del Renacimiento italiano e inglés como el Fresco (...) de Siena de Lorenzetti y el frontispicio del “Leviatán” de Hobbes. El trabajo establece que, a lo menos, tres figuras retóricas renacentistas que elaboraban imágenes con fines persuasivos servían de manera eficaz para construir abstracciones políticas, ficciones y para modificar significados de conceptos políticos. En términos visuales, el funcionamiento de estas tres figuras discursivas encuentra una explicación en la teoría de los de las imágenes que Bredekamp. El artículo concluye que, si bien la vinculación de los conceptos retóricos y visuales de ambos autores puede ser ejemplificada con el análisis de una sección del frontispicio del “Leviatán”, la viabilidad de utilizar este ensamblaje como modelo teórico para el examen de otras imágenes renacentistas depende, entre otras consideraciones, de la posibilidad de asociarlas a textos, contextos y a la comprensión de la recepción de los argumentos en una discusión específica. After discussing in the first five sections the meaning and the difficulties of the principle of relativity we present a new sel of spacetime transformations between inertial systems (“inertial” transformations), based on three assumptions: (1) The two-way velocity of light is c in all inertial systems and in all directions; (2) Time dilation effects take place with the usual relativistic factor; (3) Clocks are synchronized in the way chosen by nature itself, e.g., in the Sagnac effect. How might philosophy of religion be impacted by developments in computational modeling and social simulation? After briefly describing some of the content and context biases that have shaped traditional philosophy of religion, this article provides examples of computational models that illustrate the explanatory power of conceptually clear and empirically validated causal architectures informed by the bio-cultural sciences. It also outlines some of the material implications of these developments for broader metaphysical and metaethical discussions in philosophy. Computer modeling and simulation can (...) contribute to the reformation of the philosophy of religion in at least three ways: by facilitating conceptual clarity about the role of biases in the emergence and maintenance of phenomena commonly deemed “religious,” by supplying tools that enhance our capacity to link philosophical analysis and synthesis to empirical data in the psychological and social sciences, and by providing material insights for metaphysical hypotheses and metaethical proposals that rely solely on immanent resources. In thesecond part of the article, I consider hisargument in ``The Philosophers'' Brief on AssistedSuicide'''', which claims that the distinctionbetween killing and letting die is morallyirrelevant, the distinction between intendingand foreseeing death can be morally relevantbut is not always so. I argue that thekilling/letting die distinction can be (...) relevantin the context of assisted suicide, but alsoshow when it is not. Western Political Thought in Dialogue with Asia is a unique collection of essays that examines the exchange of political ideas between Western Europe and Asia from the Middle Ages to the early twentieth century. The contributors to the volume call for globalizing the scope of research and teaching in the history of political thought.

Introduction: This study, the first of its kind in Iran, was to assess Iranian patients’ preferences for receiving information and participating in decision-making and to evaluate their satisfaction with how medical information is given to them and with their participation in decision-making at present. Method and materials: 299 of 312 eligible patients admitted to general internal medicine or surgery wards from May to December 2006 were interviewed according to a structured questionnaire. The questionnaire contained questions about patients’ preferences regarding four (...) domains of information and their participation in decision-making. Patients’ responses were measured on a visual analogue scale graded from 1 to 10. Results: The mean (SD) score for desire to receive information was 8.88 out of 10 (1.5) and for participation in medical decision-making was 7.75 out of 10 (3). The desire to receive information was greater in women than men (9.0 (1.5) vs 7.8 (1.4), p = 0.025). It was also correlated with their education (r = 0.2, p = 0.001) and their estimation of the severity of their own disease (r = 0.13, p = 0.027). The score for preference to participate in decision-making was higher in women than in men (7.95 (2.8) and 7.0 (3.2), respectively; β = 0.8, p = 0.022) and was negatively correlated with education (r = −0.14, p = 0.015). Discussion: This study shows that Iranian patients are highly interested in receiving information about their condition and participating in clinical decision-making. No predictive variable for such attitudes was found; therefore, the only way for the physician to recognise patients’ desire is to ask them explicitly. (shrink)

Meaning in life has become an important topic in empirical research in the psychology of religion. Although it has been studied and found applicable in many different contexts, research on meaning in life and sources of meaning in African countries is scarce. This study qualitatively investigates understandings and experiences of meaning in life and sources of meaning among urban and village dwellers with different educational backgrounds in Zambia. Seven focus group interviews (total N = 52) were conducted and analysed, drawing (...) on Schnell’s model of meaning in life and sources of meaning. The results indicate that the concept of meaning in life is relevant to both urban and village dwellers in Zambia. Meaning experiences and sources of meaning are associated with certain life domains: relationships; religion; education and work; leisure activities; and health and survival. Each life domain includes several fundamental sources of meaning, which can be related to Schnell’s four dimensions of sources of meaning: self-transcendence, self-actualisation, order and well-being and relatedness. The results are discussed in light of extant studies on meaning and cultural characteristics in Africa. (shrink)
